Process:

If the food bank wants to collect certain data it can create a user defined query. For instance if the food bank wants to know how many vehicles the agency has available it can create a UDF and require agencies to provide that information. If the food bank sets it up for online submissions it will be found here in the Other Agency INFO (***ADD PICS*} If any requests are on this page click on Open Form and complete the request and then   Save. 

For example the food bank may want to know how vehicles the agency has at it's disposal, or the average number of volunteers every week.
